Qantas A380 flew to Dallas with work light left in wing


A 33-centimeter maintenance work light was left inside the wing of a Qantas Airbus A380 during maintenance at Sydney Airport on January 7, 2026, and the aircraft subsequently flew two international sectors to Dallas and back before the tool was discovered on January 9, according to an Australian Transport Safety Bureau investigation released August 17.

The ATSB found the incident resulted from a breakdown in foreign object control and tool accountability processes. Three separate safety checks—a foreign object clearance inspection, a tool return check, and a maintenance certification—all failed to catch the missing light before the aircraft was cleared for passenger service.

ATSB Chief Commissioner Angus Mitchell said maintainers did not find the light during their foreign object clearance inspections at the end of the maintenance work. “The work light’s absence was not noticed when the maintainer returned the tools, or during the unreturned tool check at the end of the shift,” he stated in the investigation report. A licensed aircraft maintenance engineer then issued a certificate of release to service despite the light being unaccounted for, and the operator’s maintenance information system did not provide a control to alert the certifier to the unreturned tool.

Foreign object debris (FOD) poses significant safety risks in aviation. According to the FAA, FOD can damage aircraft engines if ingested, cut aircraft tires, lodge in aircraft mechanisms preventing proper operation, and injure ground personnel. While the work light caused no damage to the aircraft in this case, the ATSB emphasized that such objects could move around during flight and jeopardize safety.

Qantas’s Response and Prior Incident

In response to the occurrence, Qantas issued a safety directive on February 18, 2026 to address changes to its tooling control processes and procedures. The airline has also formed a tooling working group to develop various initiatives, including new technologies, to improve tool control across its maintenance locations. A Qantas spokesperson said the company has “strengthened our tool-control procedures, including introducing a mandatory pre-clearance check to confirm all tooling is accounted for following maintenance.”

This incident marks the second time in recent years that a tool has been left inside a Qantas A380. In December 2023, a 1.25-meter nylon compressor turning tool was left inside an engine during scheduled maintenance in Los Angeles and was not discovered until January 2024, after the aircraft had completed 34 flights and accumulated approximately 294 hours of flight time, according to an ATSB report published in November 2024. That tool was found behind the fan blades of the number one engine.

CQUniversity aviation expert Steven Leib told the ABC that unaccounted-for objects found inside aircraft are “a real safety concern” because they could move during flight and jeopardize safety. However, he noted that the discovery systems worked in both cases: “The discovery of the lost object was made by the accountability system. They were able to trace when it was last used, when it was meant to have been returned and what work was being carried out.”

ATSB Chief Commissioner Mitchell emphasized that tool control systems must account for human variability. “Even well-established processes can be vulnerable when they rely solely on consistent human performance, particularly during maintenance tasks, which are often conducted in challenging environments and under operational pressures,” he said. “Tool control systems and procedures must account for this variability, with safeguards to detect and recover from errors.”
