IMSS defines rules for Mexican repatriates’ health coverage through 2026

Mexico’s Instituto Mexicano del Seguro Social (IMSS) continues to expand health coverage options for Mexican nationals returning from abroad, building on broader universal healthcare reforms announced in 2026. As Mexico transitions toward a unified health system, repatriated citizens face evolving rules for accessing social security benefits and medical services.

Mexico’s government has long emphasized support for returning migrants. In a multi-agency initiative coordinated by the Interior and Foreign Secretaries, the IMSS signed agreements to help insure Mexicans living abroad as independent workers, with consulates in the United States and Canada training staff to register migrants in the program. By joining the IMSS as independent workers, repatriates gain access to medical and hospitalization services for their families in Mexico, disability and life insurance, and disability pay for illness.

The pathway for repatriates typically involves affiliation as independent workers, which allows them to contribute directly to the IMSS system. Those who are formally employed in Mexico are automatically enrolled in IMSS through their employers, but self-employed repatriates and those not in formal employment can voluntarily enroll. The program provides comprehensive coverage including medical consultations, medicines, hospitalization, surgeries, emergency care, and specialty services.

Mexico’s broader health system overhaul, announced in April 2026 by President Claudia Sheinbaum, aims to achieve universal health coverage by 2027 by merging IMSS, IMSS-Bienestar, and ISSSTE into a single unified system. As part of this transition, all Mexican citizens were to receive a health ID card (credencial) in 2026. The government has stated that the exchange of specialized medical services between the unified system’s components will begin in the second half of 2027, with full universal implementation targeted for 2028.

Repatriates returning through Mexico’s “Mexico Embraces You” (México te Abraza) program, launched in January 2025 to support those deported from the United States, gain access to these health services alongside other assistance including financial support via the Tarjeta Bienestar Paisano card. The program has provided support to over 189,000 repatriated Mexicans since its implementation.
